The Home Health Team Leader Assistant is responsible for assisting the Team Leader in coordinating a team of staff to assure the continuity of high quality care to home health patients assigned to the team's area. This includes ensuring all team staff are skilled and informed and that they are committed to providing quality care in an efficient manner in accordance with physician's orders and State/Medicare guidelines.

Qualifications:

License Requirements
  • Current CPR certification required.
  • Current LPN licensure in state of practice Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
Additional State Requirements
  • LA- Current LPN Licensure in LA without restrictions. At a minimum, one year of experience working as an LPN.
  • TX – Current LPN/LVN licensure in TX.
  • AL, AZ, AR, CA, CO, FL, GA, ID, IL, KY, MD, MS, MO, NC, OH, OK, OR, RI, SC, TN, VA, WA, WV, WI - No additional.
