The Social Worker in Home Health assists patients and families in coping with problems resulting from severe or long-term illness, and with difficulties in recovery and rehabilitation. The Social Worker will assess, diagnose, and treat patients, mental and social conditions, counsel individuals and|or families, and update case records. The licensed social worker with a baccalaureate degree performs these functions under the supervision of the licensed master's degreed social worker.

Qualifications:

Education Requirements
  • Alabama - Must be currently licensed by the Alabama State Board of Social Work Examiners to practice social work in the state of Alabama and at least 1 year of social work experience in a health care setting. Individuals with a master's degree from a school of social work accredited by the Council on Social Work Education may practice independently. all other licensed social workers must be supervised by a licensed master's degreed social worker. Must have a current driver's license and vehicle insurance, and access to a dependable vehicle. Current CPR certification required.
  • Arkansas - Must be currently licensed by the Arkansas Social Work Licensing Board to practice social work in the state of Arkansas, and at least 1 year of social work experience in a health care setting. Individuals with a master's degree from a school of social work accredited by the Council on Social Work Education may practice independently, all other licensed social workers must be supervised by a licensed master's degreed social worker. Must have a current driver's license and vehicle insurance, and access to a dependable vehicle. Current CPR certification required.
  • California - Must have a masters degree from a school of social work accredited by the Council on Social Work Education and be currently licensed by the California Board of Behavioral Sciences to practice social work in the state of California, and 1 year of social work experience in a health care setting. Must have a current driver's license and vehicle insurance, and access to a dependable vehicle. Current CPR certification required.
  • Florida - Must have a masters degree from a school of social work accredited by the Council on Social Work Education and be currently licensed by the Florida Department of Health to practice social work in the state of Florida, and 1 year of social work experience in a health care setting. Must have a current driver's license and vehicle insurance, and access to a dependable vehicle. Current CPR certification required.
  • Georgia - Must have a masters degree from a school of social work accredited by the Council on Social Work Education and be currently licensed by the Georgia Board of Professional Counselors, Social Workers, Marriage and Family Therapists to practice social work in the state of Georgia, and 1 year of social work experience in a health care setting. Must have a current driver's license and vehicle insurance, and access to a dependable vehicle. Current CPR certification required.
  • Idaho - Must be currently licensed by the Idaho State Board of Social Work Examiners to practice social work in the state of Idaho, and at least 1 year of social work experience in a health care setting. Individuals with a master's degree from a school of social work accredited by the Council on Social Work Education may practice independently. all other licensed social workers must be supervised by a licensed master's degreed social worker. Must have a current driver's license and vehicle insurance, and access to a dependable vehicle. Current CPR certification required.
  • Illinois - Must be currently licensed by the Illinois Department of Professional Regulation to practice social work in the state of Illinois and have 1 year of social work experience in a health care setting. Individuals with a master's degree from a school of social work accredited by the Council on Social Work Education may practice independently. All other licensed social workers must be supervised by a licensed master's degreed social worker. Must have a current driver's license and vehicle insurance, and access to a dependable vehicle. Current CPR certification required.
  • Kentucky - Must be currently licensed by the Kentucky Board of Social Work to practice social work in the state of Kentucky and have 1 year of social work experience in a health care setting. Individuals with a master's degree from a school of social work accredited by the Council on Social Work Education may practice independently. all other licensed social workers must be supervised by a licensed master's degreed social worker. Must have a current driver's license and vehicle insurance, and access to a dependable vehicle. Current CPR certification required.
  • Louisiana - Must have a masters degree from a school of social work accredited by the Council on Social Work Education and be currently licensed by the Louisiana State Board of Social Work Examiners to practice social work in the state of Louisiana, and 1 year of social work experience in a health care setting. Must have a current driver's license and vehicle insurance, and access to a dependable vehicle. Current CPR certification required.
  • Maryland - Must be currently licensed by the Maryland Board of Social Work Examiners to practice social work in the state of Maryland and have 1 year of social work experience in a health care setting. Individuals with a master's degree from a school of social work accredited by the Council on Social Work Education may practice independently, all other licensed social workers must be supervised by a licensed master's degreed social worker.   • Minnesota - Must be currently licensed by the State of Minnesota Board of Social Work to practice social work in the state of Minnesota and at least 1 year of social work experience in a health care setting.   • West Virginia - Must be currently licensed by the West Virginia Board of Social Work Examiners to practice social work in the state of West Virginia and have 1 year of social work experience in a health care setting.
